우리나라는 현재 유래 없이 빠른 속도로 노인인구가 증가 하고 있는데, 2010년 통계청 자료에 의하면 2010년 65세 이 상 노인인구에 비해 2030년 2.3배, 2060년 3배 이상 증가할 것으로 예상되며(Statistics Korea, 2011), 평균수명도 길어 짐에 따라 건강하게 노년을 보내는 것에 대한 관심이 높아지 고 있다. 노화에 따른 생리적 변화는 다양하게 나타나는데 가 장 보편적인 변화 중에 하나인 근감소증(sarcopenia)은 그리 스어 기원인 muscle을 뜻하는 ‘sarco’와 감소된다는 뜻의 ‘penia’가 합성되어 만들어진 단어로 노화와 관련된 근육량 (muscle mass) 감소를 의미하며(Rosenberg, 1989), 여기서 근육이란 골격근을 의미하고 평활근과는 관계가 없다. 즉 근 감소증은 주로 사지에 분포한 골격근의 감소를 의미한다(Park, 2007). 근감소증은 진단방법과 남녀에 따라 유병률의 차이가 있기 는 하나 근육량은 주로 40대 이후 감소하기 시작해서 연령이 증가함에 따라 급격한 감소를 보이는데, 65세 이상에서는 근 감소증 유병률이 약 10%이고, 80세 이후에는 최고 50% 이상 증가하는 것으로 나타났다(Baumgartner et al., 1998; Tanimoto et al., 2012). 특히 노인에 있어 근감소증은 낙상, 일상 생활수행 장애 등을 일으켜(Scott et al., 2014) 삶의 질을 떨 어뜨리고, 일상생활의 의존도를 높임으로써(Go, Cha, Lee, & Park, 2013; Tanimoto et al., 2013) 건강한 노년을 보내 는데 장애물로 작용한다.

영문초록

Purpose: This study was conducted to estimate the prevalence of sarcopenia and its associated factors in Koreans, who were 65 years of age or older. Methods: Data were from the Fifth Korea National Health and Nutrition Examination Survey. The appendicular skeletal muscle mass was measured by a dual x-ray absorptiometry in 770 men and 1,000 women. We defined sarcopenia as appendicular skeletal muscle mass/weight*100(%) less than 2 standard deviations below the mean for gender specific young, healthy reference populations. Health and nutritional factors were compared in participants with and without sarcopenia. Results: The prevalence of sarcopenia was 6.6% for men and 9.4% for women. Multiple logistic regression analysis showed that age, cancer, hypertension, and asthma were significantly associated with sarcopenia in men. However, in women, only one factor, current job, was significantly associated with sarcopenia. Conclusion: Sarcopenia was significantly associated with age, chronic disease, and current job in Korean elders. These results suggest that chronic disease and limited physical activity should be further investigated for their association with sarcopenia.
